Re: Headend Logs sheets

We have a spreadsheet like Splicer for life mentioned. I'm thinking you are looking for a Daily, Monthly check list. The checklist may include for example fiber portion of the headend(TX/RX outputs, detector voltage, total  input power, internal/external padding, optical in/output power).

We created our own, but it's layed out based on how our headend and hubs are setup. If that's what your looking for PM me.
