I agree that extras and especially anything that requires additional hardware should not cost the installer.  However, I don't agree that the installer should make the appointment with the customer.  After 25 years in this industry, I cannot tell you how many times that I was burned by an installer making an appointment and then never following thru.  This results in a very pissed off customer that is blaming me. Appointments need to go thru the supervisor so that it can be tracked and followed up on.
