How many systems (usually bigger cities) seen the ground wire chopped as high as they could, too many.  Turned out this issue originated in the headend.  Was working on it when noticed on bbx the up snr dropped to 15 and about 30 minutes late of finding nothing it cleared up. Came back in less than an hour.  Looking at bbx I checked the node next to it, called headend and found out the two were combined together going into the cmts.  The snr problem was coming from the other node, port 5 had an elevated noise floor.  It cleared itself up and have been working on it today and believe found the cause.  Read my other post and you will see our experience with this issue and the power bars solving it.
