I that case you would have upstream noise or badly aligned returns that would cause CER/CCER at the modem or on the plant side.  This will then cause the CMTS to attempt FEC.  They are essentially the same thing in most cases.  You can have no plant noise, minimal reflections, good cpe, a good return all the way to the node, and have a bad laser or tx'er with issues that will be the root of your issues.  Do you have access to any good software that can see or compare where or what the issue(s) might be?  Just remember CCER to a point will not affect service.  CER 1% and above will.  You wll also have FEC errors at the CMTS equal to the CER that the CMTS is reporting a modem will have.  It is possible to have good SNR and get CER/FEC issues.  If this is happening look from the node to hub/OTN/headend to find that.
