If you are self employed and have no other employees you would not be required to develop and maintain Injury and Illness records. OSHA 300 & 301 See 1904.1(a)
You still would be required to follow OSHA or state safety regulations. See part 1926.16 Prime is responsible for safety and the sub contractor at any tier is responsible for his part .
